If you have been diagnosed with prediabetes or are at risk for developing type 2 diabetes, this program is for you. You can prevent type 2 diabetes. This program can help. If you are ready to make the changes needed to prevent type 2 diabetes, keep reading.
This proven lifestyle change program can help you prevent or delay type 2 diabetes.
During the 12-month program you will:
- Work with a trained lifestyle coach
- Receive support from others working on the same goals as you
- Be empowered to make healthy eating and physical activity choices
- Achieve and maintain a healthy weight
- Identify ways to manage stress
- Implement strategies to stay motivated
- Attend classes either in-person or online
Both the onsite and online programs are delivered by trained lifestyle coaches who understand the difficulty of making changes and are there to help you every step of the way. Each hour-long lesson introduces one strategy that has been proven to help prevent type 2 diabetes. Each lesson includes simple steps that you can take to eat healthy and be active. You will leave each lesson feeling empowered and motivated to make changes in your life to prevent diabetes.
Phase I includes 16 lessons focused on the basic strategies needed to prevent diabetes. These first 16 lessons will motivate and empower you to make healthier food choices, increase physical activity, lose weight and manage your stress levels.
Phase II includes at least 6 lessons that occur less often than the lessons in Phase I. These lessons provide additional support and cover specific topics such as stress and eating, maintaining weight during the holidays, heart health, healthy cooking and strategies to maintain your weight loss.
You can register to attend the 12-month program in either an onsite or online format. You will participate in the ENTIRE program in the format that you select at the time of registration.
The onsite program is currently offered in selected counties across North Carolina through providers that have been recognized by the Centers for Disease Control and Prevention (CDC) and are located in your community, oftentimes in YMCAs or local health department facilities. Most classes follow a group format that includes some discussion and physical activity.
Eat Smart, Move More, Prevent Diabetes is an online program that is delivered using synchronous distance-education technology. This means that the classes are delivered in a real-time, online format, so the participants see and hear their instructor in a live session and interact with their instructor and other class participants during the session. Participants receive support during each interactive session from the instructor and other participants, and outside of the sessions through individual online or electronic feedback from the instructor.
The cost of the 12-month program is $429 for onsite classes and $449 for online classes.
State Health Plan Members: Diabetes prevention programs are no longer paid for by the NC State Health Plan for Teachers and State Employees, which is managed by the NC Department of State Treasurer.
